How we use your health records

 

Your doctor’s practice collects information about you and the treatment that you receive when you visit to provide you with the best possible health care. This is called your ‘health record’. He or she also uses this information to help you to make informed choices about your care.
 
Your health record may be passed on to other health providers, such as a nurse or specialist at your hospital, where necessary, to ensure that they have the most up-to-date information about your health if you have an appointment with them.
 
You can request a copy of your health record from your doctor’s surgery. Please speak to the staff there for more information.
 
Your information may also be used to support the administration of the NHS. What this means is that in general, your information will be used anonymously (i.e. you will not be named) for reporting purposes. Specific details will generally only be used to support referrals for health care, for example if you are referred to a hospital or community service for further treatment.
