Report on integrated care in Greenwich

NHS Greenwich CCG  publishes our report summarising progress in delivering co-ordinated care for residents with complex health and care needs in the Royal Borough of Greenwich.  Royal Greenwich Coordinated Care - developed by the Royal Borough of Greenwich, NHS Greenwich Clinical Commissioning Group, Oxleas NHS Foundation Trust and the local voluntary sector - has already been praised by the care and health sectors, from Government ministers and from healthcare experts.  In March 2014, we scooped the Gold award in the Transformation in Heath and Social Care category at the Improvement and Efficiency Awards 2014.​ 

Our report describes the co-ordinated care system that has been established and the vision for future developments.
The initiative has already delivered co-ordinated rapid response and intermediate care services for older people and people with physical disabilities.  The next phase will extend functional integration across all services and organise health and social care delivery around clusters of GP practices.
