Long term conditions (LTC) are conditions that at present cannot be cured but can be managed through medication and other therapies. Examples include diabetes, heart failure, chronic obstructive pulmonary disease (COPD), cardiovascular disease (CVD), asthma, dementia and many others.
If you have a long-term condition, your GP can help you to manage your condition to stop it from getting worse.
With the help of your GP, specialist nurse and local support groups and you can take control of your condition and avoid trips to hospital.
Your GP or specialist nurse can help you learn about your condition so that you recognise the warning signs and avoid ending up in hospital.
